La figure ci-contre montre les principales causes de d\u00e9faillance pour chaque phase du cycle de vie.<\/p>\n<p>D&rsquo;apr\u00e8s l&rsquo;\u00e9tude du HSE, <strong>plus de 60 % des d\u00e9faillances ont \u00e9t\u00e9 \u00ab\u00a0int\u00e9gr\u00e9es\u00a0\u00bb dans le syst\u00e8me de s\u00e9curit\u00e9 avant sa mise en service<\/strong>.
